Trip Permission - Unity Walk for Israel

  • I, parent listed above, give permission for my child(ren), listed above, to travel by bus with Chabad Hebrew School to the Lag Bomer Unity Walk for Israel at the Ottawa Jewish Community Campus (Field next to OJCS, 31 Nadolny Sachs Private Ottawa, ON K2A 1R9) on May 26, 2024.

    I understand that student pickup will be at the Ottawa Jewish Community Campus at 12:00pm.
